Breezemonte Homes, Inc. has been in business since 1995, providing support services to persons with developmental disabilities in Northeast Florida. All services are provided by persons with experience, education and/or training as required by state certification. Trainings include, but are not limited to, the Core Assurances, CPR, AIDS 104 as well as training in particular developmental disabilities the staff will be encountering. Staff are screened by Local Law Enforcement and Federal Dept. of Law Enforcement.
| Services Provided: Behavior Analysis- assists a person to learn new behavior that are related to existing challenging behavior Respite Care- provides supportive care and supervision when caregiver is unable to do so Personal Care Assistance- assists with meal preparation, eating, dressing, personal hygiene and other self care activities Residential Habilitation- provides supervision and training activities to assist a person in acquiring, maintaining or improving skills related to daily living | Supported Living Coaching- provides supports necessary for a person to establish, live in and maintain a household of his/her choosing in the community Supported Employment Coaching- provides training and assistance to support persons in community jobs In-Home Supports- provides personal care, companionship and may assist in other activities of daily living Transportation- provides rides to and from person's home and their community based waiver services |
